16. Don't Step On My Blue Suede Shoes / Tritt mir nicht auf meine blauen Wildlederschuhe


Drawing lines in the sand and asking loudly for those things you want to happen is what you are going to do when you need to redefine and sensibly refurnish your own space. Four weeks in rehabilitation showed me that this is true in any clinic as well as everywhere else:

I chose that particular hospital because it expressly touted its willingness to serve not only the body but also the mind and soul. And sure enough, in almost every public lesson (an hour, once or twice per week) we - the patients - were told by the doctors about the close relationship between heart attacks, burn out and depression. Apart from other information, we were given this:

"Close to 50% of all patients who have had a heart attack slip into a depression immediately or within about three months after the event. If someone chooses to ignore this and falls under this category he increases his risk of a second attack five-fold. It is the same as if he were chain-smoking."

Does this particular clinic live up to its own standards? Not yet. While the physical check-ups and programs are fairly thorough, it takes quite a lot of effort to set up psychiatric assistance.

I should have known, though. When I asked for this service on my first day, my cardiologist said: "Well the psychosomatic department is working independently but it is relatively easy to set up a contact. But you must also know that many people here absolutely do NOT want to see that their heart attack has any relation to their psyche. That is why we normally do not offer what you are asking for in cardology."

So much for "evaluating my life situation" as I was promised in the clinic brochure. I do not ahve reliable data but I presume that the situation as such is prevalent in many clinics: Psychiatry and Cardiology on two different trains. Both know how they are related. But when it comes to cooperation: fears abound. I am convinced that this anachronism will disappear some years form now.

To make a long story short: After much sparring with different people and after myself generally being a nuisance about the whole thing (which - in light of all the non-happenings - gave me a certain satisfaction, though), I got 3 sessions of 45 minutes in a time span of nearly 4 weeks.

Vier Wochen in der Rehabilitation haben mir gezeigt, dass dies in einer Klinik genau so gilt wie irgendwo sonst:

Ich hatte jenes Spital ausgewählt, weil die Werbung ausdrücklich auf den Willen des Hauses hinwies, Körper, Geist und Seele zu behandeln. Und tatsächlich wurden wir - die Patienten - beinahe in allen öffentlichen Vorträgen (eine Stunde, ein bis zwei Mal pro Woche) von den Ärzten auf den engen Zusammenhang von Herzanfällen, Burnout und Depressionen hingewiesen. Neben anderen Informationen sagte man uns beispielsweise dies:

"Gegen 50% aller Infarkt-Patienten haben sofort oder innert drei Monaten eine Depression. Wer das ignoriert und in diese Kategorie fällt, verfünffacht das Risiko eines zweiten Anfalls. Das entspricht ungefähr dem Zweitinfarkt-Risiko eines Kettenrauchers."

Lebt die von mir gewählte Klinik ihre Standards? Noch nicht. Während die physischen Untersuchungen und Programme ziemlich gründlich sind, muss man sich schon ziemlich ins Zeug legen, um psychiatrische Unterstützung zu bekommen.

Ich hätte es allerdings wissen müssen. Als ich meinen Kardiologen am ersten Tag nach diesem Service fragte, meinte er: "Nun, die psychosomatische Abteilung arbeitet unabhängig von unserer Abteilung, aber es ist relativ leicht, einen Kontakt herzustellen. Sie müssen aber wissen, dass viele Leute hier absolut nicht sehen wollen, dass ihr Infarkt irgend etwas mit ihre Psyche zu tun haben könnte. Deshalb bieten wir das, wonach Sie hier fragen, in der Kardiologie normalerweise nicht an."


Soviel zum Thema "Evaluation der Lebenssituation", wie sie in der Klinik-Broschüre versprochen wurde. Ich habe keine verlässlichen Daten, würde aber annehmen, dass die Behandlung in vielen Kliniken so läuft: Psychatrie und die Kardiologie auf zwei verschiedenen Zügen. Beide Seiten erkennen die Zusammenhänge. Doch wenn es dann um Kooperation geht: Berührungsängste. Ich bin überzeugt, dass dieser Anachronismus in den nächsten Jahren verschwinden wird.

Lange Rede, kurzer Sinn: Nach vielen Kämpfen mit verschiedenen Leuten und nachdem ich mich ganz generell als "mühsam" erwiesen hatte (was allerdings - angesichts all der Dinge, die NICHT passierten - recht befriedigend war), erhielt ich schliesslich 3 Sitzungen à 45 Minuten in einem Zeitraum von beinahe vier Wochen.
